You need to submit the following documents: Old passport in original with self-attested photocopy of its first two and last two pages, including ECR/Non-ECR page (previously ECNR), the page of observation (if any) made by Passport Issuing Authority and validity extension page, if any, in respect of short validity passport and Proof of present address

Yes. The police verification will be done at all the three addresses where the applicant has resided in the last one year.

Yes. Both the parents need to sign to the effect that the particulars of the newborn mentioned in the application are correct and parents are responsible for its correctness.

In this case, a fresh passport has to be issued to provide this service. The application form is the same for general passport application form.

The passport is dispatched by India Post through Speed Post to the present address of the applicant.

It is advisable that the applicant himself collect the passport from the postman. In case of absence of the applicant, it is at the discretion of the postman to hand over the envelope to some family member of the applicant by checking their/applicant's ID proof or to return the passport to the issuing Regional Passport Office.

To apply for duplicate passport in case of lost or damaged passport, you need to apply for "Re-issue" of passport.

Under the new system it is mandatory for all applicants to be physically present while submitting the passport application form at the Passport Seva Kendra.
